SERVICE & SUPPORT ADMINISTRATION DEPARTMENT

The Service & Support Administration Department is the social service component of the Trumbull County Board of Developmental Disabilities. They serve all eligible individuals requesting services from birth to death. They provide the following functions:

• Ensure that Individual Service Plan (ISP) is completed to the benefit and satisfaction of the individual served.
• Ensure the plans are understood regarding expectations and desired outcomes.
• Gather and review all comprehensive information with regard to individual’s preferences, goals, needs, abilities, etc.
• Revise the plan as needed.
• Establish a budget that projects costs that will allow for a plan to be implemented and followed.
• Assist the individual served in choosing a provider.
• Ensure services are effectively coordinated and provided.
• Monitor implementation of the plan.
• Provide Quality Assurance oversight to ensure the services are delivered as developed in the Individual Service Plan.
• Ensure individual served has a designated person to provide representation.
• Establish eligibility determination for board services.
• Be single point of accountability regarding responsibilities for decision making regarding the Individual Service Plan which includes working with the team to ensure health and safety regarding Major Unusual Incidents.
• Provide staff that are available 24/7 to respond to all emergencies. The Service & Support Administration Department is responsible for taking immediate action to ensure the health, safety, and welfare of individuals.
• All Service and Support Administrators shall be required to complete training and professional growth activities necessary for maintenance of their required registration, certification, or licensure.
• It is the policy of the Trumbull County Board of Developmental Disabilities to determine available resources. The Board shall develop and implement separate waitlists as necessary. Presently there is no wait list for SSA services.
• Individuals 21 years of age or older have the right to inspect and review their records and information. In addition, legal guardians for an individual may request and review records and information on behalf of the individual.
• Employees are expected to maintain the highest possible ethical and moral standards. It is essential that Board employees do not use their public jobs for personal gain and provide any special treatment to any individual, group, or entity.

   

  
  The board, leadership, and employees of the Trumbull County Board of Developmental Disabilities are steadfast in maintaining and creating programs and services that meet the unique needs of individuals and their families.
