ELIGIBILITY DETERMINATION CRITERIA The following services are available through the County Board:
1. Service and Support Administration (planning, information, and referral, etc.).
2. Family Resource Program (respite care, socialization, adaptive equipment needs, home modification).
3. CITE Work Training Programs (SOV, Riverside, Harding, Reserve, Opt-4 Cite).
4. Adult Services (Sheltered Employment, Community Employment/Follow Along Services.
5. Residential Options (Supported Living, Waiver, ICF/MR).
In order for a person to receive services from the Trumbull County Board of MR/DD, a person must meet the eligibility criteria:
1. Must be a resident of Trumbull County.
2. Must have documented disability, a physical or mental impairment other than mental illness.
3. The disability must have manifested prior to the age of 22.
4. The disability is likely to continue indefinitely.
5. Completion of an assessment called the Ohio Eligibility Determination Instrument (C/OEDI). A person must have documented substantial functional limitations in several areas of the instrument.
